Jones, Kaluta, and Wrightson. I haven't seen a copy of this book since it was first published. I remember at the time asking the owner of the LCS if there was any new material in it at all, and he just sneered down at me and said, very condescendingly, 'Of course there isn't, that's why it's called 'Deja Vu' - already seen, all reprint - don't you understand?'
