Picked this up recently, yellowing on cover, and I believe printer holes on bottom. Well, except for the one closest to the spine. I'm guessing a Q 6.0

Looks better than the raw 5.5 I used to own. I'd give it a 7.0 if it weren't for that LLHC, but I can't see the yellowing all that well. I'll say 6.0, maybe 6.5

Qualified is used to denote one (or so) large defects that would keep an otherwise higher-graded book from scoring the big number. You might have a book that is otherwise 9.8 but someone took a Sharpie and drew an "X" from corner to corner. You could go with a blue label and get a big ding, or go green and they would grade it ~9.8 with the qualification that the big "X" on the cover is what keeps it from scoring that grade in Universal.

 

That's just one example.

The grabber holes does not detract from grade, as they are part of the manufacturing process. If the wear is not much more than what’s visible, I’d surely give it a 7.0 or so, but the bottom corners wear could bring it down a bit, so FN+ 6.5?
