Batman 298 DC Comics Questionnaire insert 3 3

Bought a group of 12 books on eBay and found the 298 to have a three-page DC Comics questionnaire in the center. Anyone know of the rarity? I've never come across one in my collecting. Same bunch had a book with the Atari insert. That's one I've known about but not owned till now.

I did notice there is a copy on eBay right now for $48. Any info would be great.

I sold a Justice League with one of these last year for around $60 on eBay, and I bought a lot of Detective Comics that had a 476 (one of my favorite comic covers and stories) that had this insert. It doesn't seem to be as talked about as Mark Jeweler's variants, but there are collectors out there who go after these.
