I'm thinking a 4.0/4.5 as is. A C&P should bring it up to a 5.0. The torn page should have no effect on the grade.

4.5 imho, with the amount of color breaking creases on the back and Thumb/reader creases on the front it looks borderline to me, might get 5.0 on a good day
