The detached staples and the long tear detract heavily, but the overall decent eye appeal keeps it from falling below 3.0.

No... It would just get a grade and detached cover would be noted on the label.

 

Qualified grades are typically only assigned when a book presents as a ceratin grade but there is one overriding flaw that is not typical for the apparent grade. For instance, a book that looks 9.4 but a page is missing.

 

A detached cover is consistant with a lower grade book such as this, so no need to qualify it.
