X-Men 94 CGC 8.5 WP.  Bought as an 8.5, had pressed and cleaned (with much improvement) and came back as an 8.5 again.  Beautiful book.

4) No returns on slabs - you'll get the actual slab pictured, packed securely.  If USPS still manages to destroy it somehow, that's what the insurance is for - i'll make certain you're made whole no matter what.  (Although that's yet to happen - I use a LOT of bubble wrap.)
