From Hell HC A/P

This is an osbcure one, but here goes. The HC for 'From Hell' was originally shrink-wrapped. However, some of them were Artist's Proofs. How did the book publisher designate that the book was indeed an Artist Proof?

 

As a more general question on Artist's Proofs of HC (which do crop up from time to time on ebay), how are they designated if they are in the original shrink-wrap?
