What was the entire contents of the first MMM mail in kit?

Can somebody link me, or list all the the items that one recieved if they bought the first Merry Marvel Marching Society mail in fan kit back in (1967?)?

 

I bought a MMM record awhile ago, and was wanting to get the whole original set to go with it.

 

And was wondering if anyone here currently has it, or bought it way back when..
