Manufacturing Error - the book isn't correct, but the error isn't on the part of a collector (who might have cut out a coupon or pulled out the centerfold and then tried to put it back), the error is from the manufacturer. Is that a manufacturing error as well? Generally speaking, the qualified label is used when it's possible to have this discussion: "That shouldn't be there. Is it on all the copies?" "Yes" = blue label "No" = green label (This is also true for things like missing coupons... "There's a hole on this page, that shouldn't be there.") Strangely, they also recognize a Venom Lethal Protector #1 White Error, even though only 1 or 2 of them have ever been found. They're not consistent on this topic, but I think we can agree that a blue label would be preferred, so there are times when CGC "upgrades" the green label to blue.
