PS Form 2976-A-PMI

I'm trying to send some books to a boardie in Ireland. I've printed a (PS Form 2976-A-PMI) from PayPal and took it to the post office and they said I'm missing a page. I've reprinted it but it's still only has two pages. Can someone tell me how many pages this is supposed to have.

That's the USPS customs form for an international package, right?

 

It prints on two sheets of paper, but you then have to cut each of those in half, so you end up with 4 half sheets - one of those you keep (they're marked at the bottom), the other 3 you sign and put in the 2976 clear plastic envelope which attaches to the package.
