Sorry for the delay (man's gotta eat)...

 

I just looked at it (15) again and it has a diagonal crease (faint) on the front cover (bottom right) and one on the back cover (same corner). Bright colors. I can see it as 5.5 to 6.0. I'll do a full scan tomorrow (scanner at the office) of the front an back of both.

 

And yes, shipped in a box.
