CGC modern Submission form?

Hey everyone. Just wondering if anyone had a pdf cgc submission form they could send me. Hit me up via pm. I'm not entirely positive if this is the right place(i doubt it is honestly) but any help would be greatly appreciated. I am trying to do a mail in submission for some books. Thanks in advance.

Ya sorry links won't work, you have to log in under your membership, then go to submission centre and your all set.

You can access a PDF form, fill it out and print it at cgccomics.com... Just log in and off you go.
