Daredevil Annual #4. They originally released one in 1976, then stopped doing annuals for a while. They started them back up in the 80s and mistakenly numbered the next issue #4 again.

I should clarify one thing-

It wasn't a sequencing problem as concerns issue numbers-

It was a sequencing problem within the *particular* issue (#4)as concerns pages.
