How can I get a Creator Agreement Form to grade a signed comic?

I have a Venom comic with original art and signed by artist Ryan Stegman. I have access to him but I was told I need a specific form for him to fill out. Could you please direct me to where I can get said form, please? Thank you.

Good morning, thanks for your message.  You would need to send your request to our Signature Series department and they can provide you with more information.  Please email them at signatureseries@cgccomics.com.
