Online sub form bugged? 0

So i have been trying to fill out another sub form to ship in and when i click the link i either get a blank white screen or a grey screen with bubbles saying loading then an error message. This happening to anyone else?

Thanks for your message.  Our server was down for a good portion of the afternoon yesterday but it should be back up and working this morning.  Please let us know if you still experience an issue.  Thanks!
