Carol Danvers first appeared in Marvel Super Heroes.  Ms Marvel first appeared in Ms Marvel 1.

Glen, Regarding Mark Bagley I thought his first published Spider-Man art was Amazing Spider-Man Annual 22? A year earlier July of 1988. Great update
