Attempt to request new set type keeps getting prevented by error message 1 1

Hello CGC admins,

I have attempted to request a new set type in Sports Cards and keep getting an error message when pressing the submit button.  Please see the screenshot below.  I have closed my browser (Google Chrome on Windows 10Pro) and re attempted the request submittal but receive the same error.
