Here ya go. Roughly issues # 30 through # 50 could fit your bill. # 45 is especially horrible.

It's a heart-breaker that an otherwise very cool Ditko & Wood collaboration has become the ubiquitous odd-ball book of the 1960s... I have come across collections containing literally 100s of high grade copies of this book.
