Yes, this seems quite strange the piece would come off inside the slab

I wonder if the chip was there ready to fall off before the grading, or if it occurred after being sent in for grading.

Yes, It certainly wouldn't hurt to at least discuss with Harshen or somebody else from CGC to see what they can do.
