His conversations with the "doctor' definitely feel sort of like a schizophrenic talking to himself. I could see that too but... that would mean he never had the brain surgery and he only subconsciously got "better"? No, I think something did happen to him and that doctor might have been someone he used to know during his "evil" days. Another thing, the doctor was shocked that he contact the police and I am waiting to see how this plays out after he tried to turn himself in. There are other players in this twisted book.
