Cover is attached, more firmly than it looks.

Beacause of the small piece missing around the top staple, I'd give it a 3.0 to 3.5.

Beacause of the small piece missing around the top staple, I'd give it a 3.0 to 3.5.

 

Just to clarify, I realized it may be a little hard to tell from the pictures: There is a piece missing to the left of the top and bottom staples, but the cover is still attached at the staples. The paper quality on the top 2/3 of the splash is darker, while the rest of the book is a off white - cream color.
