I'm not sure that's a smoke stain. It looks like an especially heavy oxidation or 'dust' shadow (which isn't actually caused by dust). Pity, because it's otherwise a nice book.

 

Given that CGC don't seem to hit shadows like that too hard, I'd hazard a guess that your book might still score a fine(ish) grade--5.5 to 6.0. Others might know better than me though.

Whatever the stain is, it's pretty dark. The books looks structurally solid otherwise, but I'd have a hard time considering it any more than a 4.0/4.5 with that notable a stain. CGC may disagree.
