3) In one panel in Avengers #2, the Hulk's left foot is drawn with only 3 toes instead of 5.

 

Hulk has only three toes on the cover (on both feet) of both Avengers 1 & 2 yawn. That was how Jack wanted him to look.

I think in one of the PPSSM issues of the "Death of Jean DeWolf" story on page 1 Spidey is seen swinging in a center panel - only there's no web in his hands. When Peter David signed my copy he opened it up and drew the web line in.
