This scan's too small to grade I guess, but here it is anyway. I was looking around for something to send in with my trivia contest free standard submission, otherwise I wouldn't have bothered to send it in. I'll post the slabbed version after we hear some guesses. There is a 1" horizontal tear on the left edge of the back cover, with a piece of tape (sinner!) on the inside back cover where the tear is.
