What issue features Rob Liefeld's infamous Captain America panel?

I wanted to collect his wonderfully crafted Heroes Reborn series.

 

I believe he did the first six issues. I bought 2-6 today but that shot was nowhere to be found.

 

Is it in issue #1?

Pretty sure it's a promo shot or advert and wasn't featured in any of the issues of the '96 run, not if my memory serves me correctly anyway.

I'm going to go with this: Liefeld drew the torso, then he drew the shield. He was going to place the head where the added head was realized that didn't fit with the shield, and rather than have to redraw everything, just kinda put the arm and head in there to tie the pieces together.

 

A bigger mystery is how this ever saw the light of day.
