I believe it was given out at one of the annual Diamond Distributors forums along with a Gold copy of Cerebus #0. I've got one.

I did actually. I don't remember who from, but yeah. It was definitely Heroes.

Yeah, that makes sense. Although, I've seen several of the Gold 0's, but they haven't had this one with it. It's gotten me thinking that the gold 0's were also distributed some other way too in addition to the way you describe sans the mini book.

I think the golds were originally premiums that were given out to stores based on the number of copies they ordered. I don't know the ratio. But, I assume they had some extra Golds and packaged them with the Ashcan 201's. I would guess there were at least 1000 of those sets that were made, but that's just a guess.
