What grade would this get? 0

I bought my first artist sketch cover but the back cover has black spots on it. If the rest of the comic were a 9.8, how much would these marks cost me in CGC grades?

Adding ink to the cover is what it's made for. Even if there's an 'error' while sketching, such as these ink spots, it's not CGC's place to determine what does or does not belong on the blank.

Edit: It should not be seen as a defect. I have to imagine that CGC agrees.
