sent my submission form instead of the one it says to send with submission

I sent wrong submission form with my comics I sent the one that’s for me and not the one that says to send with order will I have an issue with my submission being submitted because of this or will it be OK since both forms are almost identical? my copy and the submission copy

Hello,

There will be no issues with receiving this submission. The main thing that is needed is the CGC order number and the barcode at the top of the page. Let us know if you have any other questions.
