Obviously a book is graded on its entirety, with all defects taken into consideration to determine a final grade, as far as a "loose/detached centerfold", a completely detached centerfold is only allowed in grades GD 2.0 or lower (possibly a GD+ 2.5) any higher grade book will take a hit for a completely detached centerfold, how much of a hit, depends on the overall condition of the book. IMO, a VF/NM 9.0 will take a bigger hit than a VG/FN 5.0, however, there is nothing in the grading standards that is black and white on how much of a hit a higher grade book takes for 1 defect that is not allowed in the grade, there are however, lots of grey areas in grading and much is left to the opinion of the grader at the time the book is being graded.

FN/VF 7.0 - VF 8.0 - Centerfold is mostly secure.

FN 6.0 - Centerfold may be loose.

VG/FN 5.0 - Centerfold may be loose.

VG 4.0 - Centerfold may be loose or detached at one staple.

GD/VG 3.0 - Centerfold may be loose or detached at one staple.

GD 2.0 - Centerfold may be loose or detached.

FR/GD 1.5 - Centerfold may be loose or detached.
