The staple placement on this book is just plain ugly.

I know this is common for older books but have you ever seen such a thing on an almost modern age book?

Would you say it's been pressed and that's the cause.

Or simply a factory defect?

It's an incredibly common defect on comics of virtually any age.  Staple placement and fold/centering are something that doesn't show up in grades (accept for maybe the highest grades), but is an important consideration to many collectors.

While it's possible it's been exaggerated by pressing, typically, this is a production issue as mentioned.

Quite the miswrap for this book as well.  Is there a spine roll?
