Metarog has kindly pointed out that the ultra rare giveaway, which I've never seen in my life, a DC from the mid fifties called "WHAT DO YOU KNOW ABOUT THIS COMIC'S SEAL OF APPROVAL", has had one copy slabbed.

My understanding is that in some cases, CGC scans covers of books for a nominal fee, if the submitter requests it. Not sure how or whether you can access such a scan if it exists...?

It's not a myth. I saw one for sale in a mail order auction catalog (years ago) and if I remember right the cover is mostly text (of the title).
