You can tell it's real. It's a smart bit of body paint, someone was paying attention to metallics. And the pointillism is great.

Mouth is outlined in something like eyeliner. Teeth could easily be rubbed. You're an artist, c'mon--use your imagination.

Mostly I'm wondering how she got the space between lower lip and teeth black since this is just air with a tongue behind it.

Edit-close up reveals top of lower lip is blackened

Exactly. Her lips are closed and the black and teeth areas are painted on.

 

It's really obvious when the photo is enlarged.
