Pre-Screen Form? What Am I Missing 0

Hey, all -- I should know this because I've submitted a pre-screen before, but for the life of me I cannot figure out how to qualify my CGC submission with a pre-screen grade. 

Can someone help me out here? Do I just write it on the packing slip?

After you enter in 25 books on the form, that option will pop up.
