I am not sure on exactly how these were distributed, but they were made and do exist.   Even if they were not intended on being available to the general public.  Similar to the early Red "Sample" labels when CGC first started.

Curious, so I tried this specific book and yup, no GP custom label shows.  Does for other books (non Avenger/JLA) so it looks like a bug on the form.  You can always complete the form and then hand write you want to add the GP custom label and they will adjust the charge.
