But to answer your question, it shouldn't be a big deal if it doesn't affect the story or artwork. Something like this usually gets a note on the label, though. And depending on how nice the rest of the book is, the grade reduction could be minor or fairly significant...maybe even a Qualified label if the book is extremely high grade...

Welcome to the boards!

If it was a manufacturing defect, it should not affect the grade.   I have a book with siamese pages. First 8 wraps were not cut properly. The reader couldn't open them so the siamese pages were broken but all stories were not affected.  That is one I am not sure about the grade for breaking the siamese pages.
