Is 'what if featuring planet hulk' the first app of skaar. Why is it not listed on the cgc label if it is.

Because what's on the label does not matter and does not change the contents of the comic.

Not every 1st appearance is listed on the label in every case.  
 

but more importantly, this is more likely a question for a cgc representative.  
 

If you want to know why a given company does or a doesn’t do very specific thing, consider contacting the company.

it is noted on the cgc label for World War Hulk #5. Skarr appears at the end of the book coming out of a pool in a single panel. This should be a cameo instead as it is very much like Hulk #180 except Skarr has no dialogue nor is he named. I agree with you, it should be the What If.
