Spectacular Spider Man #76 -- Lakeside Tattooz variant 0

I just acaquired this book

I am currently pressing it and will be sending it in.  This has an envelope inset with the tattoo samples in it.  I guess some are there and some aren't.  Will it be on the notes that the tattoo is included?  Should I put a note in the bag with the comic?  I know this is a little off the wall... I can send picks if that helps.

This book has special notes that are added to the labels regarding these tattoos. If the insert is included then there will be a note stating: "Tattooz" insert. But if this insert is not included in the book the label will note: "Tattooz" Insert. ("Tattooz" missing from insert). If this is missing, we do not downgrade for this though. I hope this helps!
