After seeing the OP, Jeff, is from Western NY and was setup up at a three-day coin show, I tracked him to the Gold and Silver Expo 2022 in Cheektowaga, NY. The show was scheduled to end at 3:00 and I got there around 2:30. Couldn't find Jeff as people were packing up but eventually I saw the W.B. Mason box from his original picture. We had a good 45 minute chat.

My mistake. The books were signed, but the signatures were not noted on the label.

I think that's more likely than 2 different version of a cover getting pulled. Good call.

I don't know if we discussed it here but I did talk to someone in the printing business and he said it's possible but improbable. Covers are manually loaded into the bin for assembly. Someone would have had to have loaded the Direct covers on top of the newsstand covers (not out of the ordinary) and the bottom cover of the Direct pile would have had to somehow stick to the top newsstand cover. I've got a 100 Easy Crossword Puzzles cover attached to a Sgt. Fury book. Anything is possible.

Not a quality control issue, but an issue nonetheless.
