I don't think there's anything that has been published that we know existed at one time, but no longer exists...lots of stuff that was supposed to be published...but I don't think there's anything that's been published that we know existed, but now doesn't.

Also - Visions #1 had a printing error, what the publisher referred to as a "Drafting diagram" on the Flaming Carrot story. Lamar Waldron says that he "trashed" all known copies of these errors (about a total of 200), but that was after at least one of them was found in a convention

Agree with this as it was my first thought.  Definitely any 11, that said I know there are several books that have been chased , but are now assumed never produced.  Right off the top of my head.
