BangZoom
Were there any 2 page spreads that were part of a story? If so, which was first? The earliest I can think of is Bill Barnes # 7 (July 1942). Can anyone else think of one earlier?
Point Five
Some of the Timely guys should chime in. I'm pretty sure some of the earliest Captain America books (1941-ish) had centerfolds that were full single panels.

I know they were especially prevalent through the issues in the twenties, but those would have been late '42 and into '43. Almost double page splash in CA 7 -- there are some small panels at the bottom. Definite one in 8. A couple other almosts after that. It's been too long since I've read these -- great Simon/Kirby stories and art.

Scrooge
... and then I guess this is an example of what you're calling an almost-double-spread page - (from Cap # 10) -

lou_fine
Didn't most of the early Centaur books have similar type double page centrefold spreads such as this one. Of course, they were really more of a pin-up type as opposed to being a part of an actual storyline.
adamstrange
they were really more of a pin-up type as opposed to being a part of an actual storyline. That's why, as beautiful as that is, I did not include it in the list. I don't think there's anything too special about comics having a large illustration (that is, many magazines/pulps/books etc had two page illustrations) but it does seem interesting to me when artists starting stretching out and taking advantage of the unique aspects of the comics medium. Battery is charging on the camera, so I will post the rest of the story later today. I got you covered Bill. I posted the splash earlier and kept on wondering why you uploaded it again since it should have been in BZ's checklist until I realized that the Funny Pages appearance is a reprint from the Detective Picture Stories from April 1937 where this story originally appeared and that BZ has listed. It's the first recycled full page splash. I wonder if Bob Kane insisted on getting paid again for the reprint!! Anyway, to answer Ryan, here's the rest of the story. Now I have to go ahead and actually read it ...
