I've had that happen too. The only solution is to require immediate payment. That's what I do. And I state that I will combine shipping, and refund any excess paid. Soon, eBay will be implementing a checkout system, where people can add 30 of your items to a checkout, which will use your combined shipping rules, and they pay for it all at once. Until then, I'll just run the risk of a multi-item buyer not wanting to pay for the extra shipping until I can refund it.
