Promo at gaming stores like Funcoland....as far as I know. I have a copy I got from a guy who used to manage one.

I bought a NM one for $40 or so off ebay earlier this year.

 

It reprints like half of maximum carnage part 1.

But, if you remember how large game boxes were back in the day, it could have fit inside one....

No. The game was released for Super Nintendo and Genesis; the boxes the game came in are therefore about 5 x 7 .

I had that game but recently sold it at a flea market but still have the cassette tape that came with it. The tape was a pretty cool collectible.

Earthbound for the SNES is a tough game to find. Insane prices when you do.
