Miracleman breaking Kid Miracleman's neck when he reverts to human form to escape the pain of a rock and a girder being materialized into his head and chest by Aza Chorn (second place bonus to Chorn for being pushed too far and not pointlessly throwing things but rather "embedding" them).

Cue Ben wearing a copy of his original helmet from when they began, again, for about a year's worth of real-time issues, after which he just openly left the scar-tissue exposed. He and Wolverine ultimately met for drinks and made up, spurred on by Reed's 'death'. His face ultimately got returned to normal as a 'gift' from Hyperstorm, the villainous son of Franklin Richards and Rachel Summers from the DOFP continuity (I think, the exact continuity may not have been confirmed). So all up it lasted from issue 374 to around 405 or so, not bad in comic bad terms for a 'real change', particularly the 90's.
