From the photos, I can see that the entire back of the card has staining, which heavily effected the surface grade, and therefore the overall grade of the card. There also seems to be paper pulls on the top Pokémon logo. Paul

No. These aren't cards that are getting the old labels now. These are cards that were holdered in old labels before the change and are being shipped out. The old labels were just fine, but the new ones are of course an improvement. However, we would only offer a free reholder if the grade was printed outside of tolerances on the old label. Paul

This is correct. There will still be some old labels trickling out on slower tiers. Paul

I am sorry to hear that this card went down in grade, but our standards are simply different from other companies'. As for the damage, please note that before you are allowed to enter card information on a crossover, you must agree to this waiver which pops up. In the future, I suggest simply opening the holder yourself so you are able to better inspect the card in person. No matter the grade or the qualifier, it is still an awesome card! Thank you, Paul

Oh no! That is unfortunate. While of course there is always the possibility of damage occurring when cracking out of any case (and a waiver is needed to submit for crossover), it is exceptionally rare as the people removing the cards from the holders do it all day every day. They are pros. I would chalk this one up to a difference in grading standards. Paul

Cards, even those straight from a pack, are not guaranteed to be a ten. Paul

Well it is good to hear that the newer inserts are performing better. This issue is still pretty rare overall, and only seems to happen when the boxes are really mishandled by the shipping service. I do not believe we are using the older inserts anymore, but there still might be submissions going out in the older ones as we finished up the stock of those. My suggestion would be to give Plast-x a go as that would be easiest/quickest for you. If that doesn't help, or they are too severe, please contact customer service for options.
