For what it's worth, I'm fairly sure that no one intended anything other than implying that Rob Liefeld can't draw feet. Or bodies, for that matter... At the end of the book, Batman embeds a batarang into Spawn's skull. Think they did some Spawn books where he had stitches in the middle of his head as well.
